Employees and self employed Persons who work in a country other than their resident country (e.g. frontier workers from Germany who work in Switzerland) plus non-active family members living with them.
 

 


Attestation of entitlement of the insured to benefits in another country than the one in which they are normally granted insurance benefits in cases of disease. Serves for registration in the resident country.

Persons who are entitled to benefits in kind in another than the competent state.
 

 


Notification of suspension or discontinuation of the claim for benefits in kind.

For family members who are residents in another country than the employee or the self- employed.

Note: The regulations on assistant benefits in kind for family members of a person insured in a Swiss health insurance but living in an EC member country, is normally defined by the regulations of the assistance institution in the resident country.

European Health Insurance Card (EHIC)

 


Tourists, students, delegated workers, unemployed persons, travellers on business

 


The EHIC is issued for insured persons who will temporarily stay in an EC/EFTA member country, for example on a holiday, a business trip or for a semester of studies. It has to be presented to the medical service provider in case of immediately necessary medical treatment which cannot wait until the intended return to the home country. Every insured person will receive an EHIC in his/her own name.
In case the insured person is not able to present the EHIC on time, for example because of loss or late delivery, a Certificate provisionally replacing the European Health Insurance Card will be issued by the competent health insurance.
