Application and request for exemption online

Please submit us online your complete information to the compulsory insurance.

We guarantee a decision within three working days!

Before you send your data, please prepare following the documents in scanned form.

1. Border workers in the cantons of Aargau, Appenzell Ausserrhoden, Basel-LandschaftBasel-Stadt or Glarus as well as in the cities of Zug, Cham and Risch Rotkreuz who want to take a health insurance in Switzerland

  • Registration form (You will find the registration form on the side of each canton. Please note only your name, first name and date and sign the form. We will need your signature for legal reasons)
  • Border worker’s permit (front and reverse side)
  • A current insurance policy with EU/EFTA premiums
  • Form “Choix du système d’assurance maladie applicable” (absolutely necessary for persons with residence in France)

2. Border workers in the cantons of Aargau, Appenzell Ausserrhoden, Basel-LandschaftBasel-Stadt or Glarus as well as in the cities of Zug, Cham and Risch Rotkreuz who want to be exempted from compulsory insurance in Switzerland

  • European Health Insurance Card (EHIC) (compulsory insurance)
  • Confirmation / stamp and signature of the private insurer (private insurance)
  • Form “Choix du système d’assurance maladie applicable” (absolutely necessary for persons with residence in France)

3. Residents in the cantons of Aargau, Appenzell Ausserrhoden, Basel-LandschaftBasel-Stadt or Glarus as well as in the cities of Zug, Cham and Risch Rotkreuz

  • Request for exemption (Please note only your name, first name and date and sign the form. We will need your signature for legal reasons)
  • Residence permit
  • European Health Insurance Card (EHIC) (compulsory insurance)
  • Confirmation / stamp and signature of the private insurer (private insurance)
  • For people in education: Training contracts, certificate of matriculation or an education acknowledgment
  • For people with a other residence purpose: Reasons for exemption (please find details on page 3 of the information sheet form the application for exemption)

4. Pensioners with residence in the EU/EFTA

  • Request for exemption (Please note only your name, first name and date and sign the form. We will need your signature for legal reasons)
  • Confirmation of reception of health insurance in the State of residence or confirmation of the future inclusion
  • Confirmation of deregistration of the residential community in Switzerland
  • Pension approval(s)
  • Form “Choix du système d’assurance maladie applicable” (absolutely necessary for persons with residence in France)
